Prognostic Significance of the AgNORs, Cathepsin-D and Heat Shock Protein-27 in Breast Cancer

Abstract
The breast cancer has not been a major health problem and has not received public attention up until recent years when the prevalance of breast cancer soared up after cervical and gastric cancer among Korean women. In general, fifty percent of
the
patients with operable breast cancer will benefit from surgery alone or in combination with radiation. But the remaining of patients need adjuvant systemic therapies, namely chemotherapy and hormone therapy, which carry a number of complications,
unfortunately Therefore, the evaluations of the patients with high risk for recurrence is important to identify those who could potentially benefit from adjuvant systemic therapy or do not need additional therapy.
The AgNORs, Cathepsin-D and HSP-27 have been suggested as prognostic indicators of breast cancer. To evaluate the usefulness of the AgNORs, Cathepsin-D and HSP-27, as prognostic indicators of breast cancer, 57 cases of breast cancer were selected
for
this study during five-year period starining was used for estrogen receptor, HSP-27 and Cathepsin-D and argyroiphilic staining for AgNORs.
The estrogen receptor was positive in 37 cases (64.95). The mean unmber of AgNORs was 4.6 in breast cancer. The AgNORS had no correlation with age, number of positive axillary lymph node, tumor size and estrogen receptor status .Cathepsin-D was
overexpressed in 61.4% of total cases, 55% of postmenopausal cases, 75% of lymph node negative cases, 76.9% of cases which size of mass was less than 2cm and 64.9% of estrogen receptor positive cases. The disease free survival rate was
significantly
increased in overexpressed group of Cathepsin-D (p<0.05). HSP27 was overexpressed in 38.6% of total cases, 35% of postmenopausal cases, 40% of lymph node negative cases, 69.2% of cases which size of mass was less than 2cm and 45.9% of estrogen
receptor
positive cases. The disease free survival rate was significantly increased in overexpressed group of HSP-27 (p<0.05).
As a result of this study, it is concluded that the AgNORs is not a useful prognostic factor but the Cathepsin-D and HSP-27 appear to be useful in predicting the prognosis for breast cancer.
